Fall sports registration times announced at Cazenovia Central Schools

— Attention all Student-Athletes: In order to make the process of clearing student athletes for the 2013 fall sports season more efficient, the Cazenovia Central School District nurses will be available to collect required student health information and sports registrations during three days in August - August 15, 16 and 19. The school nursing staff will be available on the field at equipment handout to collect required forms and issue “Green Cards” for JV and Varsity football players on Thursday, August 15, at 5:30 pm.

All other sports team athletes may choose to attend “Green Card Days” on Friday, August 16, from 8 am until noon, or Monday, August 19, 8 a.m. until 2 p.m.

Student-athletes in grades 7, 8 and 9 may report to Mrs. Belton in her Middle School Office. Student-athletes in grades 10, 11, 12 should report to Mrs. Siracuse in her High School Office.

Please be advised student-athletes and parents, these are the only opportunities available to receive your “Green Card” clearance. No student-athlete will be allowed to practice without a Green Card. Green Cards may only be issued by the school nurses.

Attention all modified football players; there will be a parent meeting on the evening of August 19. Mrs. Belton will be available to collect all paperwork for Green Card clearance.

Required forms to play a sport are: a current physical with the health history portion completed (see back page of physical form) performed in the last 12 months by a duly licensed health care professional, an Interval Health History Form, and the Extracurricular Registration Form signed by a parent.

The required forms can be obtained from your coach or printed off the School website at caz.cnyric.org/athletics/sign-up-forms/.

We strongly encourage that a parent accompany their student athlete to the Green Card registrations to be sure all paperwork is filled out and handed in.
